The virtual patient self-assessment for COVID-19 will assess recent travel, signs and symptoms, and community exposure. This will also capture information about a participant’s age, chronic comorbidities, and risk. Once a patient has been assessed, the patient will receive a standard program providing education, monitoring, and access to a health navigator. Patients are matched with a health navigator who will provide monitoring, education, and communication. Health navigators are the first line of communication for general questions and keep the population informed. If care escalation is required, the patient will be triaged to an RN who will support the patient, and ensure that the patient is tested at an assessment centre if needed.

This solution has the ability to conduct population health reporting, case management, and monitoring on patients those who are infected, those at risk, and those in treatment. Patients can report on signs and symptoms of COVID-19. Providers can monitor and report on signs and symptoms, allowing providers to report on people who have been infected and people around them who should be checked. Providers can be proactively alerted when the nature and severity of a patient’s respiratory symptoms reach dangerous parameters. This will flag changes in temperature and symptom severity to identify clusters of the population who are at elevated risk and recognize participants who require additional support.

In addition to monitoring and reporting, NexJ Connected Wellness will provide patient education, care coordination, and coaching to support the population health at scale. Patients will be connected with a Health Navigator and Nurse as needed. Patients can also be connected with family, friends and other loved ones for a meaningful, pragmatic, actionable support structure during social isolation. Secure chat and video chat on NCW allows patients to communicate with their Health Navigator or Nurse for guidance and advice. With community forums, patients can share their experiences and support one another. Patients can anonymously ask and answer questions, and providers can push information to targeted groups. Patients can also be informed with the health library on NexJ Connected Wellness, which provides trusted educational content including prevention guidelines, monitoring signs and symptoms, what to do if you suspect you’re infected, preparation and guidelines for self-isolation and quarantine. NexJ’s coaching support programs include: Managing COVID-19, Stress Management and Mindfulness, Cognitive Behavioural Therapy, Managing Chronic Disease, and Healthy Living.
